Hyponatremia Correction


1. Determine how slowly serum sodium concentration (Napre) should be increased over 24 hours (eg, 1440 minutes). Namely, choose the new, desired serum sodium concentration (Napost).
2. Setup a sodium gradient (difference) between the CVVH replacement fluid and prevailing serum sodium concentration.
3. Calculate the fractional increment of the gradient that yields the desired serum sodium concentration. Use accepted guidelines to determine the desired serum sodium concentration.
4. Analogize kinetic urea clearance to sodium dialysance to determine the fractional change in the sodium gradient.
