Your health care team is made up of:
A Nephrologist is a doctor who is trained in internal medicine with a specialty in kidney disease.
Registered Nurses specialize in dialysis treatment. They can teach you about the advantages and disadvantages of Peritoneal Dialysis as well as train people to do dialysis themselves.
A Social Worker can help you and your family with understanding how to live with chronic illness. The social worker provides counseling and education and works with you to develop the necessary resources to cope with being on dialysis.
A Dietician is someone trained in diet planning and nutrition. Your renal dietician has specialized training in kidney disease and will work with you to create a customized diet plan that works best for you.
You are the most important member of the health care team. After training from the team you will feel ready to be the master of your own dialysis care. You will learn to perform dialysis at home, check your blood pressure and weight, and eat a healthly high protein diet. You will also learn when to contact your doctor or nurse with any problems or concerns you're having at home. As your caregivers, we will work hard to answer your questions, calm your fears, provide support to you and your family as well as provide quality medical care. You must be an active participant in dialysis in order to get the most from your treatment.
