The prevalence of CKD — Chronic Kidney Disease — in the United States stands at approximately 20 million persons. In addition, Wayne County has the highest prevalence of any county in the nation. Therefore, the prevalence of CKD is greater than that of diabetes regionally and nationally.

Optimal management of patients with kidney disease requires early disease recognition and collaboration between the primary care physician and nephrologist. Because multiple terms have been applied to CKD patients (chronic renal insufficiency, chronic renal failure, pre-endstage renal disease, endstage renal disease), the National Kidney Foundation has replaced all of these terms with the more encompassing term, CKD, which encompasses the entire continuum of kidney dysfunction.

Dedicated time for CKD patients involves the skillful practitioner’s concentration to delineate the myriad tasks that are necessary to maintain the patient’s health, his speed to partner and delegate the various responsibilities of care to the TEAM that he develops, his ability to focus on the more serious tasks first, and ultimately, to execute the plan.
